Annual consolidated audited financial statements, management letter, response to the management letter, and annual activity report.

S igned original complete AFCP Program Delivery Funding Applications.

AFCP Reporting Schedule (special agreement)

June 15 - each year
Interim unaudited consolidated Balance Sheet, an Income Statement, and a related Activity Report for the two months ending May 31, 20XX in order to obtain the second quarterly payment.

September 1st - each year
Annual consolidated audited financial statements, management letter, response to the management letter, and annual activity report.

September 15th - each year
Interim unaudited consolidated Balance Sheet ending August 31, 2004, an Income Statement ending August 31, 2004, and a related Activity Report for the Months of June, July and August in order to obtain the third quarterly payment.

November 15th - each year
Interim unaudited consolidated Balance Sheet ending October 31, 20XX, an Income Statement ending October 31, 20XX, and a related Activity Report for the months of September and October 20XX in order to obtain the final quarterly payment.

December 1st - each year
Original signed complete AFCP Funding Application to PTA
